About the Role As a Remote Advanced Clinical Practitioner, you will work autonomously to provide advanced clinical care to patients across all Archvale practices. You will use your expertise to deliver high-quality care through remote consultations, triage, adhering to clinical guidelines and contributing to the effective management of the practice. Key Responsibilities Delivery of highly effective, patient-centered care through remote synchronous and asynchronous consultations. Performing remote triage and prioritising patient needs effectively. Management of acute and long-term conditions, including medication reviews. Processing administration tasks promptly, including referrals, repeat prescription requests, and other related documentation. Maintaining accurate, up-to-date clinical records in line with best practices and policy. Supporting the achievement of practice targets, including QOF indicators. Participating in audits, significant event reviews, and quality improvement initiatives. Collaborating with multidisciplinary teams to ensure coordinated and continuous care. Contributing to the development and implementation of clinical protocols and guidelines. Participating in appraisals, revalidation processes, and continuous professional development. Promoting a culture of safety, learning, and continuous improvement within the practice. Qualifications/Requirements Registered healthcare professional (e.g., Advanced Nurse Practitioner, Paramedic Practitioner, Pharmacist) with an Advanced Clinical Practice qualification. Independent prescribing qualification. Minimum of 3 years’ experience working as an Advanced Clinical Practitioner in a primary care setting. Advanced IT competence and experience using both consulting and communication software. Proven experience in remote triage and consultation, including synchronous and asynchronous methods. Experience in remote clinical assessment. Ability to manage referrals, clinical notes, and other administrative tasks accurately and promptly. Strong interpersonal skills to deliver clear, empathetic communication with patients and collaborate effectively with teams. Ability to thrive in a remote working environment, demonstrating flexibility and adaptability to challenges.
