I have already drafted the full document in Microsoft Word and now need the dropdown logic wired in so that only the information tied to the chosen option appears; everything else must stay hidden. The dropdown values will come from a predefined list I will supply, and the revealed content should be presented as form fields, ready for users to fill in.

You are welcome to review the file as it stands today—I will send it over along with the exact list that drives the dropdown. Your task is to embed the controls, set up the conditional formatting or field-based rules, and return a clean, ready-to-use Word template plus a brief note explaining how you implemented it so I can maintain it later.
