Mobile-Friendly Dog Walking Forms

I have a detailed booking agreement sitting in Google Docs and I need it transformed into a clean, on-brand experience that clients can complete easily on their phones. The same form must live in three places and look consistent everywhere:

• A responsive online version embedded on my current WordPress site
• A fillable Google Doc (for internal use and quick edits)
• A fillable, downloadable PDF

Please style every version with my existing logo, colour palette, and font choices so the form feels identical to the rest of my website. After a client submits the WordPress form, they should receive an automatic email confirmation that summarises their answers and lets them know we’ve received the booking.

I’m open to the plugin or form builder you prefer, as long as it integrates smoothly with WordPress and keeps the load time light. The finished assets should be fully mobile-friendly, tested across common browsers, and delivered ready to plug straight into the site.
