Step 1: Take MANDATORY Photos (Do this FIRST!)
Turn ON the GPS/Location on your phone camera. You MUST take clear, geotagged photos of:
The outside of the main university building at ul. Świeradowska 43, 02-662 Warszawa.
A clear, readable signboard showing "Uczelnia Łazarskiego" or "Lazarski University".
The inside (main reception, Dean's Office / Dziekanat Wydziału Medycznego, or administrative area, if allowed
Step 2: Request Form Completion
Show your LOA and the candidate's diploma to the authorized person (Registrar, Dean's Office staff, or University Administration).
Politely request them to fill out and sign the Verification Form
Step 3: If They REFUSE to Fill the Form
Do not argue.
Politely ask: "Could you please provide the Name, Job Title, and official email address of the person who can verify this directly with our team via email?"
Check the email: Ensure it looks like an authentic, official university email (e.g., ending in @lazarski.pl or @lazarski.edu.pl, not a random Gmail/Yahoo address).
Step 4: If They REFUSE to Give an Email Too
Do not worry! Your visit STILL COUNTS as successful.
As long as you have the mandatory geotagged photos (Outside, Signboard, Inside), you have proven you visited the correct location.
Just write a short note in your report: "Visited the location. Staff refused to fill the form or provide contact details. Geotagged photos attached as proof of visit."
Step 5: What if the Location DOES NOT EXIST? (Crucial Step)
If you arrive at ul. Świeradowska 43, 02-662 Warszawa and it is an empty lot, a residential house, a completely different business, or a virtual office with no sign of "Lazarski University":
Do NOT leave immediately. You must prove you went to the exact address provided.
Take 7 to 10 geotagged photos of:
The exact building/entrance at ul. Świeradowska 43.
The business or building that is actually there.
The immediate left and right neighboring buildings/shops.
Conduct a vicinity check: Ask 2–3 nearby neighbors, shop owners, or building security guards: "Do you know if 'Uczelnia Łazarskiego' (Lazarski University) is located here, or if they recently moved?"
Write down their answers.
Write this note in your report: "Location does not exist at this address. Verified with neighbors/security. Geotagged photos of the exact address and surrounding area attached as proof of visit."
Your visit STILL COUNTS because you proved the address is invalid/unoccupied by the stated university.
