Fluent Form Layout Field Update

I’m looking for a quick but thorough refresh of my existing Fluent Forms intake form. Right now the layout feels cluttered and the flow is confusing, so submissions are dropping off. The main objective is to clean up the design, move a few elements into a more logical order, and introduce several custom fields that are specific to our services.

Here is what I expect once the job is complete:
• A reorganised, responsive layout that guides visitors smoothly from top to bottom
• Unneeded fields removed and all new custom fields added with correct labels, help text and conditional logic where appropriate
• Colours, spacing and font sizes adjusted so the form fits neatly within our current page design
• Full testing to confirm every field validates correctly and the submission process works without errors on desktop and mobile
