Printed Docs to Excel

I have a batch of printed documents that need to be keyed into a structured Excel workbook. Every field on the page—names, dates, numeric values, and brief text notes—must be transferred exactly as shown, preserving spelling, punctuation, and any leading zeros.

The task is pure data collection and input, with no cleaning or analysis required on your end; accuracy is the only priority. Each row in Excel should mirror one line of the original form, using the column headers I will provide.

I will supply high-resolution scans (PDF) plus a small sample file illustrating the expected layout. Once you finish a section, send the sheet back so I can spot-check before you continue; this rolling review helps avoid any re-work later.

Deliverable
• Completed Excel file containing every record from the printed documents, ready for immediate use.

Acceptance criteria
• 100 % of fields transcribed.
• Less than 0.5 % transcription error rate (I will audit a random sample).
• Workbook returned in .xlsx format, no macros.
