PDF-to-Excel Patient Data

I have a collection of patient information currently stored in individual PDF files and I need each record transferred accurately into an Excel workbook. The Excel sheet will serve as the master database for our medical billing workflow, so absolute precision on names, birth dates, policy numbers, and contact details is essential.

You will receive the PDFs, an example row showing the exact column order, and brief notes on a couple of fields that require standardized formatting (e.g., date style). Simply open each PDF, extract the data, and type it directly into Excel—no other software is necessary. If anything in a file is unreadable or missing, flag it in a comments column so I can follow up with the clinic.

The finished deliverable is the completed Excel file returned back to me, ready for import into our billing system.
