Paper Form Data Entry

I have a collection of paper forms that need to be converted into a digital dataset. Each form contains a mix of text fields and numerical values, and I want every detail transcribed exactly as it appears. Because the information is handwritten, I am relying on careful manual entry rather than any automated extraction.

You will receive high-resolution scans of the forms. Your job is to type the contents into the spreadsheet template I provide, keeping the original field order so it aligns with our internal database. Consistency in spelling, punctuation, and number formatting is essential, and any unclear characters should be flagged for my review rather than guessed.

Deliverables
• Completed spreadsheet with 100 % of the records entered
• A short log noting any unreadable entries or discrepancies

I will be available to answer questions quickly, and I expect the same level of communication if anything needs clarification. Accuracy is more important than speed, but once you have a rhythm we can discuss a realistic timeline for the full batch.
