Organized Medical Excel Sheet

I need a clean-looking Excel workbook that lets me store and view two key datasets side by side: patient information and each patient’s medical history.

For the patient section, all records have to sort automatically in alphabetical order by last name, even after new entries are added. Typical columns will include full name, patient ID, DOB, phone, and email; feel free to suggest any other essentials that make retrieval easier.

The medical-history section must line up with the matching patient and display events strictly in chronological order. I want room for date, diagnosis, procedures, notes, and optional follow-up reminders. A simple drop-down to tag the visit type or severity would be helpful but not mandatory if it complicates things.

Please set up user-friendly data-entry forms (native Excel forms or a little VBA—whichever is quicker) so staff can’t break the structure. Basic error checks—like preventing duplicate IDs or impossible dates—will keep the file reliable.

Deliverable: a ready-to-use Excel file with sample rows that demonstrate the sorting logic and data validation, plus brief instructions inside a separate sheet so anyone in the clinic can start using it right away.
