Interactive Google Sheets Service Report

I have a service report already laid out in a static file and now need the same look and flow rebuilt in Google Sheets so my team can interact with it online. The sheet must do more than mirror the visuals—it should streamline daily use:

• A built-in Google Forms (or in-sheet entry section) that feeds the report, so technicians can submit jobs on their phones without touching the formulas.
• Smart conditional formatting that flags missing signatures, overdue next-service dates, or readings that fall outside tolerance.
• Automatic calculations—totals, percentages, and any carry-over figures—running reliably in the background.
• Data-validation rules on every editable cell to keep entries consistent (drop-downs for status, numeric limits on gauges, date pickers, etc.).

I will provide the original layout plus sample data; you deliver a polished, share-ready Google Sheet with protections where needed and clear instructions for adding more rows in future. Once it matches the existing design and all interactive features test out correctly, the job is complete.
