I have a backlog of patient medical records that must be coded and entered under the ICD-10 standard right away. Some charts are still on paper or scanned PDFs, while others sit in an EHR export; a portion can be pulled in automatically, but every record needs a manual check to guarantee accuracy. The work therefore calls for a hybrid approach—your preferred mix of automated import/OCR tools plus meticulous manual verification—so the final dataset is 99 % accurate and fully compliant.
Speed is critical: the first batch should be back within 48 hours and the full set completed as soon as humanly possible without sacrificing precision. Throughout the project I will provide clarifications on ambiguous diagnoses and quick feedback on sample uploads.
Deliverables
• Clean, ICD-10–coded file of all patient records (Excel, CSV, or direct DB import)
• Short reconciliation log noting corrections and edge cases
• Confirmation that all data‐security and confidentiality steps were followed
If you already work with recognised coding software, OCR utilities, or scripts that accelerate the process, mention them; if not, strong manual skills are fine as long as each code and modifier is double-checked.
