Flawless PDF-to-Word Novel Conversion

I have a novel in PDF format that I need transcribed into an editable Word document so I can begin my editorial passes. Every character, chapter break, header, and page number must carry over without a single deviation—the goal is exact accuracy, not “close enough.”

Beyond perfect text fidelity, the finished .docx must mirror the original layout: identical margins, paragraph spacing, indents, italics, boldface, page headers, footers, and section breaks. In short, when I toggle between the PDF and the Word file, they should look like twins, only the latter is fully editable.

Please use whatever combination of OCR, manual proofreading, Adobe Acrobat, and Microsoft Word you find most efficient, but build in your own quality-control pass so I am not hunting for dropped quotation marks or misaligned chapter titles.
