Fix Laravel LIS Downloads & Reports

My Laboratory Information System is built on Laravel 8. Two issues are hurting day-to-day use:

1. Any zip bundle that users generate from the reports section takes far too long to start downloading. The slowness is the same in Chrome, Firefox, Safari, and on both desktop and mobile, so it is clearly server-side or code-level rather than a client quirk.

2. Each time a patient report is rendered, a row of test results repeats itself. The duplication shows in the view and in the final PDF export, which is confusing for clinicians.

What I need you to do:
• Trace and eliminate the bottleneck that delays zip file downloads (storage, buffering, headers, queue, or a misconfigured Nginx/Apache rule—whatever is responsible).
• Locate and patch the logic that pulls data for the patient report so every entry appears exactly once.

Acceptance criteria:
– A typical 5 MB zip generated from the portal begins transferring within two seconds on a 50 Mbps connection.
– A refreshed patient report shows no repeated rows and matches the underlying laboratory record in MySQL.

The project lives on a private Git repo, and I can spin up a staging branch for your fixes. You may use any profiler, Laravel Telescope, or database query logging you prefer; just keep the toolchain inline with Laravel conventions so I can maintain it later.
