Employment Discrimination Legal Paperwork

I need help preparing appeal paperwork for a judicial review of my case, which the Florida Commission of Human Resources dismissed. I was forced into a constructive termination based on age discrimination and harassment by the owner of the company. The case was dismissed due to lack of jurisdiction because the number of employees at the time of the incident was less than 15. In contrast, there were between 15 and 16 employees in the USA at the time of the incidents. Not less than 15. (Notice of Dismissal Attached)

Requirements:
