Elementary Student Interest Questionnaire Creation

I need a concise, engaging questionnaire that I can attach to my elementary school application form so I can better understand each applicant’s interests and strengths beyond grades or test scores.

The form should:
• be age-appropriate for children roughly aged 5-11;
• focus on general topics about the child—hobbies, favourite books, preferred learning activities, creative outlets—rather than drilling into single subjects like Math or Science;
• use simple, friendly wording that encourages honest, thoughtful answers;
• fit comfortably on one to two pages and mix short answer and multiple-choice items for variety;
• conclude with an open prompt that lets children share anything else they feel is important.

Deliverable: an editable document (Google Docs or Word) containing the final questionnaire, ready for me to brand and print. A brief note explaining the rationale behind the question set would be appreciated so I can tweak it confidently later.
