DWG Revisions From Annotated PDF

I have an AutoCAD DWG that already captures the bulk of the project, but my latest PDF print-out is covered with handwritten notes. Those notes dictate two things: first, several existing elements must be modified to match new dimensions and alignments, and second, a handful of items have to disappear altogether.
