Custom StepMania Chart Creation -- 2

We are looking for custom StepMania charts for our own audio tracks.

All songs require the following:
* 3 difficult levels - Beginner, Easy, Medium
* Both DDR-style single-pad (4-panel) and PIU-style single-pad (5-panel)

Output should be in .sm format, charts can use steps, holds and jumps.

Please provide samples of previous charting work. We'd like to start with a single song, then we can do songs in batches of 3. Attached are previously completed charts as a reference for the style and technical requirements.

Please quote per song price, and all bids must include a link to previously created StepMania charts.
