I’ve just launched a small site purchased through GoDaddy and I need a polished form that will power a short market-research survey. Because I’m not entirely sure what sits under the hood—GoDaddy could be running WordPress, its own builder, or something else—I’m relying on you to choose the most practical way to embed the form so it loads quickly and looks exactly like the mock-ups I’ll share in a few slides.

The survey combines open-ended prompts, single-choice questions (approx. 4 questions), and numerical inputs (approx. 10 input cells). Numerical fields must accept both whole and decimal numbers, plus percentages limited to 1–100. If a respondent types anything outside those rules the form should flag it instantly, not on submission.

When the user hits “Submit,” I’d like the responses stored in a format that’s easy to export (CSV) and an email confirmation sent to me. The form must work on mobile and desktop, and be simple for me to tweak later if I add questions.

• Fully styled, responsive survey form integrated into my existing new website
• Front-end matching my slide deck
• Real-time validation for decimals, whole numbers, and 1–100% fields
• Automatic data capture (CSV) plus email notification on each submission
• Brief hand-off notes so I can edit questions without touching code
