Build an Automated Organization Intake System with Forms, File Uploads & n8n Integration

We are developing an existing software platform and need a freelancer to build a simple, reliable automated organization intake system.
This is not a request to rebuild our existing backend or business logic. Your task is limited to creating the intake layer that collects structured information and documents from a client organization and hands them over to our existing system in a predictable format.
The intake should allow a user to:
enter basic organization details;
provide a Polish KRS registration number where applicable;
complete a structured questionnaire about the organization, its activities, areas of operation, experience, resources, team and other relevant information;
upload the organization’s statute / governing document as PDF;
upload optional additional documents;
submit everything as one complete intake package.
The output must follow a predefined structured data contract called ORGANIZATION_INTAKE_V1. This is only an internal technical name and does not affect the implementation. We will provide the exact schema before development.
The solution should include:
a clean and simple web form;
required-field validation;
KRS format validation;
secure document uploads;
deterministic submission IDs;
structured JSON output;
predictable file organization/storage;
webhook or API handoff to our existing system;
compatibility with n8n workflows;
clear success and error states;
protection against accidental duplicate submissions where appropriate;
basic logging/audit information;
simple technical documentation.
We are open to a pragmatic solution using n8n, low-code/no-code tools, a lightweight frontend, or another maintainable approach.
We do not need a large custom web application. Reliability, simplicity and maintainability are more important than visual complexity.
Important constraints:
Do not redesign our existing database unless explicitly agreed.
Do not modify our existing analytical or business logic.
Do not add AI interpretation of uploaded documents or questionnaire answers.
Do not infer information that the user did not provide.
The intake component should remain independent from the rest of the analytical system.
The final input/output contract must be deterministic and documented.

User → Intake Form → Validation → File Uploads → Structured Intake Package → Webhook/API → n8n / Existing Backend
