I have a questionnaire drafted in Microsoft Word that I now need as a smart, pre-fillable PDF. The structure, branding, and overall look must stay intact, but every answer area has to work digitally:
• Text inputs: respondents should be able to type directly into the form.
• Checkboxes: any selectable option must toggle cleanly.
• Calculations: a handful of fields need to add or average values automatically so results appear the moment data is entered.
Once complete, the document has to be fully functional on the latest Adobe Reader and remain print-friendly if someone prefers hard copy. I would like you to return:
1. The finalized fillable PDF with all interactive elements and calculations tested.
2. A brief note explaining where calculations sit and how they were set up, in case I need to tweak formulas later.
