USPTO Patent Revival Filing

My U.S. patent has lapsed and I want to bring it back to active status as quickly and cleanly as possible. I already have the patent number on hand; what I need is a practitioner who knows USPTO practice inside out and can walk me through the exact filing procedures required for revival.

The main task is to prepare a complete Petition for Revival package and spell out the steps I must take in Patent Center (or EFS-Web) to get it accepted.
