USA Doctors NPI Database

I need a complete, up-to-date list of licensed doctors/clinics/hospitals practicing anywhere in the United States, delivered as a clean Excel workbook. For every doctor, the sheet must at minimum include:

• NPI number
• Full name (first, middle, last, suffix if any)
• Primary practice address (street, city, state, ZIP)
• Mailing address if different
• Phone number and taxonomy code when available
• Date the record was last verified

I will run look-ups and filters directly inside Excel, so each field needs its own column and must be free of merged cells or hidden characters. Please pull the information from authoritative public sources such as the CMS NPPES registry or comparable datasets and note the retrieval method in a brief read-me tab.

Before the final hand-over, share a small sample so I can confirm the layout and data quality. The finished file should be ready for immediate use, with no additional cleaning required.
