Urgent PDF Forms to Word Conversion

I have a PDF made up entirely of forms that include check-boxes and drop-downs. I simply need every page reproduced in Microsoft Word so the layout looks identical and all the text can be edited. The check-boxes, dropdowns, and any other interactive features do not have to remain functional—static visual replicas are fine.

Please keep all fonts, spacing, and page breaks exactly as in the original PDF, with the form fields clearly visible so a user could type or place an “X” manually if they wish. Accuracy in formatting is more important than preserving interactivity.
