Update Online Order Form Fields and Admin page

I already have a functioning online order form plus a small admin panel that manages the submissions. I now need an experienced developer to jump in and adjust several of the current input fields—no new ones, just refinements to what’s there. The edits could include tweaking labels, changing how data is validated, or even switching a field’s type if that ends up being the cleanest fix; I’ll confirm the exact list with you once you review the code.

These frontend changes must stay fully in-sync with the admin area, so whatever you alter on the form needs to be reflected in the database handling and in the admin interface where we view and edit orders. Please make sure nothing breaks in the process: keep the form responsive, preserve existing styling, and test thoroughly before handing it back.

Tech stack is PHP and MySQL with a sprinkle of vanilla JavaScript on the client side, so you should be comfortable working in that environment and version-controlling with Git.
