I have several statute-based documents that need to become fill-able, double-sided forms in Google Docs. Each form must accept mixed data—both free-text entries and numeric inputs—while keeping the layout clean and professional with nothing more elaborate than standard formatting.

The job is straightforward: recreate my existing pages so a user can type directly into clearly marked fields on screen, then print or save the finished form front-and-back without losing alignment. All page breaks, margins, and field positions should remain consistent when converted to PDF or sent to a printer set to duplex mode.

Deliverables
• A separate Google Doc for each statute form, fully editable and shareable
• All fields formatted for easy tab-through entry (text boxes, number boxes, check marks where appropriate)
• A quick test PDF of each file to confirm two-sided alignment and fill-ability
• Brief notes on how to duplicate or tweak the template for future statutes

Acceptance criteria
Forms open in Google Docs with no add-ons required, allow mixed text/number input, print correctly on both sides, and match the original wording verbatim.
