Your task is to open each linked form, capture every field exactly as shown, and place the information into a structured spreadsheet (Excel or Google Sheets, whichever you prefer). The sheet should keep the original question order, with separate columns for text remarks and numeric values so I can run quick filters and basic statistics once the file is returned.

Accuracy is more important than speed; I will run spot-checks against the live links before approving the file. Please make sure that:

• All numbers retain any decimals or percentage signs.
• Blank responses remain blank—no placeholder text.
