Styled Word Notepad Template

I need a Microsoft Word template that looks and feels like a real notepad. Each page should open with the headings “Date”, “Title”, and then flow into a ruled “Notes” section so I can type directly on the lines without the layout shifting. The lines must stay perfectly aligned when the file is printed or when additional pages are inserted.

Please build it as a reusable .dotx file so I can create new documents from it at any time. Keep all formatting locked in, including margins and line spacing, and make sure text automatically continues onto a fresh, identical page once the first one is full.
