I’m preparing an application for a state Health grant and want a skilled grant writer to take the lead on the full proposal package. The program we’re targeting funds care-coordination initiatives, and every section has to show how our model will translate into measurably improved patient care across our service area.

You will work directly from my background notes, data sets, and prior reporting, then shape them into a persuasive narrative that aligns with the latest state guidelines. That includes weaving in evidence-based practices, outlining a realistic work plan, and presenting a budget narrative that clearly connects requested funds to expected outcomes.

Key deliverables
• Complete grant narrative (all required headings)
• Logic model and timeline illustrating care-coordination activities

• Compilation of attachments (letters of support, organizational charts, resumes)
• One revision round after state pre-review comments, if issued

Experience with state-level health proposals—especially those centered on coordination of care—will be invaluable. If you have secured similar awards before, let me know the funding agency and year. Familiarity with common tools such as Grants.gov Workspace or state portals is a plus, though not mandatory.
