About the project
Our client, a US-based digital health company, is building a closed-loop wellness platform. The central engineering challenge is integration: consolidating around a dozen existing production systems (EMR, wearables, labs, billing, messaging) into a single unified experience, automating data transfers that are manual today, and picking canonical sources of truth per data type — all while handling Protected Health Information (PHI) end to end.
We're looking for a Senior Backend / Integration Engineer to build the API and integration layer that powers the platform, working closely with the architect, mobile, and web teams in a distributed setup.
Requirements
5+ years of backend development experience, with strong hands-on TypeScript/Node.js
Proven experience designing and building API integrations with third-party systems (REST/webhooks, auth flows, rate limits, retries, data reconciliation)
Strong experience with PostgreSQL — data modeling, queries, and performance
Experience with cloud and serverless environments (Cloudflare Workers/Hono, or Node on AWS/GCP)
Solid understanding of building reliable, testable services with automated tests
Experience with background jobs, queues, and scheduling
Excellent spoken and written English communication skills
Ability to work independently and collaborate effectively within a remote or distributed team
Nice to have
Healthcare / HIPAA experience — building PHI-handling services with audit logging, retention, and vendor BAAs (strong plus)
Experience with wearable/health-data aggregators (Terra, Vital) or EMR systems
Familiarity with Stripe, managed auth providers (e.g. Clerk), and transactional messaging (email/SMS)
Experience integrating LLM/AI features under compliance constraints (BAA-covered inference, logging, guardrails)
Experience with a TypeScript monorepo (Turborepo) and shared packages
Responsibilities
Build and maintain the API and integration layer connecting the platform to external source systems
Automate data flows that are currently manual, and implement reconciliation across systems
Design and implement PostgreSQL schemas and canonical data models per data type
Implement PHI controls — audit logging, retention, and data segregation — across services
Build background jobs, queues, and scheduled tasks for ingestion and syncing
Collaborate with the architect, mobile, and web teams on API contracts and data flows
Write automated tests and own the reliability of your services
Take ownership of your part of the work and communicate clearly within a distributed team
