Revamp PDF Client Questionnaire

I have an existing fill-in PDF that serves as a questionnaire for my clients, but it needs an all-around upgrade. The current layout feels cluttered, some questions are unclear, and a few extra sections have to be added so respondents can give richer information without getting confused.

The rework needs to allow for future easy additions and/or edits.

Your task is to take the original file, streamline its design, make every field truly fillable (tab-order, character limits, dropdowns where sensible), and polish the wording so each question is unmistakably clear. If a new section is the best way to capture missing details, please incorporate it while maintaining a consistent style.

I must be able to talk to you on the Zoom or other application. There are too many nuances.

I normally edit in Adobe Acrobat but I’m open to you rebuilding the document in InDesign or another tool if that yields a cleaner, more professional result—as long as the final deliverable is a single, fillable PDF that opens flawlessly in standard Adobe Reader on Windows and macOS.

Deliverable:
• Revised, fully interactive PDF questionnaire, ready to share with clients.
