Retype Novel into Word

I have a complete novel that exists only in hard-copy form and I need every word carefully retyped into a single Microsoft Word document. No intricate layout work is required—plain, clean paragraphs with standard margins and automatic page breaks will be fine.

Accuracy is critical: punctuation, spelling, chapter breaks, and any italicized or bolded text should match the original exactly. When you finish, the Word file should be ready for me to review, edit, and ultimately send to my editor without further clean-up.
