Virtual Rockstar is hiring a full-time Remote Patient Monitor on behalf of our client — a rapidly growing home-based healthcare organization headquartered in Arkansas. This role is ideal for a compassionate, detail-oriented professional with a nursing background and strong Athena EMR experience who’s passionate about improving patient outcomes through remote care.
As the Remote Patient Monitor, you’ll be responsible for tracking patient health data, conducting outreach calls, and ensuring accurate documentation within Athena EMR. You’ll monitor key vital sign parameters, check for out-of-range readings, and coordinate with providers to ensure timely interventions when needed.
This role is a blend of clinical insight and administrative excellence — perfect for someone who understands patient care but thrives in a structured, data-driven environment.
You’ll play a key role in supporting patients receiving home visits, helping manage their ongoing health data and coordinating communication with their clinical care team.

Responsibilities
Patient Monitoring & Communication
Review and document daily or weekly patient readings such as weight, blood pressure, and glucose levels
Call patients to follow up on vitals, medication changes, or symptoms
Ask guided clinical questions to assess patient condition and compliance
Identify abnormal or out-of-range values and escalate them to the provider team
Maintain a compassionate and professional tone with patients, primarily elderly individuals receiving home care
Data Entry & EMR Documentation
Enter, review, and update patient data accurately within Athena EMR
Create, modify, and maintain templates in Athena for consistent reporting and follow-up documentation
Log all communication, notes, and observations for provider review
Track and report trends in patient vitals to support proactive care management
Team Collaboration & Administrative Support
Communicate with providers and medical staff using Microsoft Teams for updates and escalations
Coordinate with the care team to ensure each patient’s remote monitoring aligns with their visit schedules
Assist in developing workflow efficiencies and monitoring reports for quality assurance
Requirements
Nursing background (LPN, RN, or equivalent education strongly preferred)
Experience using Athena EMR is required
Knowledge of normal vs. abnormal parameters for common vital signs
Strong written and verbal English communication skills (clear, professional, and patient-friendly)
Comfort with phone-based patient communication, particularly with elderly patients
Excellent attention to detail and accuracy in documentation
Reliable technology setup with secure internet connection suitable for HIPAA-compliant work
Ability to work collaboratively and independently in a remote setting
