Real-Time HSE Reporting App

I need our current Safety, Health and Environment report turned into a live-updating application. The goal is for the HSE team to log and track three data streams in real time: safety incident reports, health & inspection records, and environmental monitoring data.

Key workflow
• The same team must be able to enter new information on a phone or tablet in the field, on a computer at their desks, or by uploading an existing report file.
• As soon as a record is saved it should sync to a central dashboard, so the whole team can see the latest status without refreshing spreadsheets or emailing versions back and forth.

Core requirements
• Simple, responsive form designer mirroring the fields in our current report.
• Secure sign-in restricted to our HSE personnel, with the option to add more roles later.
• Instant search, filter, and export of any record set (PDF or CSV).
• Push or email alerts when an incident is logged or when follow-up actions are overdue.

Deliverables
1. Working cross-platform app (web or hybrid mobile is fine) deployed to our server or cloud account.
2. Source code and clear setup documentation.
3. Brief user guide that lets the HSE team start entering data immediately.
