PowerApp Checklist with Previous Values

I need a simple Power Apps form that walks a user through a checklist of fewer than twenty questions. The list mixes Text input, Multiple choice, and Yes/No fields, so the control types must switch cleanly as the user moves down the form.
Only the Text input questions should reference history. For those, I want the most recent submitted value to appear next to the field where the new answer needs to be submitted, giving users an at-a-glance reminder of what they entered last time without overwriting the editable box.
The finished app has to save each run as a new record while still being able to pull the prior value for the relevant text fields when the same user—or any user, if that is easier—opens the checklist again. As long as the experience looks tidy on desktop and mobile and the data connection you choose is something standard in Power Apps (e.g., SharePoint, Dataverse, Excel), I am flexible.
Please build, test, and share the package so I can import it directly into my tenant and start using it right away.
