The PHRN Coordinator is responsible for conducting comprehensive reviews of patient care documentation in accordance with established standards, protocols, and regulatory requirements. This position plays a key role in maintaining the quality and accuracy of prehospital patient records. The PHRN MDS Coordinator will collaborate closely with clinical staff, managers, and other stakeholders to identify areas for improvement and provide feedback for ongoing education and training.
MDS Assessments – Complete and submit accurate Minimum Data Set (MDS) assessments within required timelines
Care Planning – Develop and update individualized care plans for residents
Regulatory Compliance – Ensure assessments and documentation align with Medicare/Medicaid, CMS, and state regulations
Interdisciplinary Collaboration – Work with nurses, therapists, and other healthcare team members to support quality patient care
Data Accuracy & Reporting – Maintain precise documentation to support reimbursement and compliance
Apply knowledge of PDPM (Patient-Driven Payment Model) to support accurate reimbursement classification
Review patient records and clinical documentation for completeness and accuracy
Identify missing, incomplete, or incorrect information and follow up accordingly
Provide feedback and support ongoing education/training based on documentation review findings
Manage multiple deadlines independently in a remote work environment
Requirements
Current and valid PHRN license
Must have bedside/clinical experience
Must have at least 2 years experience with MDS (Minimum Data Set)
With extensive team lead / supervisory experience
Has experience supporting a U.S. Skilled Nursing Facility (SNF)
Strong knowledge of prehospital patient care protocols, standards, and regulatory requirements.
Excellent attention to detail and analytical skills.
Effective communication and interpersonal abilities.
Ability to work independently and collaboratively in a team-oriented environment.
Commitment to continuous learning and professional development.
Willing to work on shifting schedule.
