PDF Literature Assessment Template

I need a reusable, middle-school-friendly assessment template delivered as a polished, fill-able PDF that I can print or share digitally. The layout must accommodate three question styles—multiple choice, short answer, and a longer essay response—without feeling cluttered for students in grades 6-8. Number each item clearly, leave generous writing lines, and include answer bubbles or tables where they make marking easy.

Flexibility is important. Some tests cover a single chapter, others revolve around broader themes, so the header area should let me label the assessment either way (“Chapter ___” or “Theme: ___”). A place for book title, student name, class period, date, points per section, and an overall score box will keep everything organised.

Please add a final answer-key page that mirrors the numbering of the main sheet for fast grading.
