Patient Treatment Data Entry

I have a backlog of patient treatment records that must be transcribed into plain text files quickly and accurately. Each record includes dates, medication names, dosages, procedures performed, and follow-up notes. Your job is to take the source material I provide (PDF scans and handwritten notes) and re-enter every detail word-for-word into the text template I will share.

Accuracy is critical—spelling of drug names, dosages, and dates must match the originals exactly. Please keep the order of entries intact so that each patient’s treatments remain chronological. All work must comply with standard patient-data confidentiality practices; no information may be reused or shared outside this project.

Deliverable:
• One cleaned, proofread text file per patient, named with the patient’s unique ID.
