I need to turn a backlog of paper and PDF patient files into a clean, searchable spreadsheet. All information goes into a single Excel or Google Sheets document—whichever you prefer—as long as it stays consistent.
The scope is strictly patient records and contact details. For every entry you will capture: name, date of birth, insurance information, and up-to-date contact details (phone, email, address). Nothing clinical or sensitive beyond that is required right now, so the task stays straightforward.
You will receive the source files in batches, along with a template that already contains the required column headers. Accuracy is critical; spelling, date formats, and insurance identifiers must match the originals exactly. I will spot-check after each batch, and any discrepancies will need to be corrected before the next hand-off.
Deliverable
• Completed spreadsheet with every patient transferred and fields fully populated.
If you have a strong eye for detail and can keep data confidential, this should be a smooth project.
