I need a fully editable Microsoft Word (.docx) template that my service desk can fill out on the spot whenever we finish an oil change. The visual tone should match a professional auto-shop—clean lines, subtle use of color for headings, room for a small logo in the upper left, and clear section dividers so it prints neatly on standard letter paper.
Essential information the layout must accommodate:
• Customer & vehicle: name, phone, email, address, make, model, year, VIN, license plate, mileage in/out.
• Service details: oil brand and type (conventional, synthetic, blend), viscosity, quantity added, filter part #, any additional services or notes, technician name.
• Financials: itemised price for oil, filter, labour, shop supplies, line for tax, subtotal, total, payment method, balance due/paid.
Deliverable
1. One unlocked .docx file containing all the above placeholders formatted with tables or text boxes so fields stay aligned when data is typed.
2. A short “read-me” page (can be a second sheet in the same file) explaining which areas are safe to edit and where our logo should be inserted.
Acceptance criteria
• Prints perfectly on 8.5" × 11" with no content running off the page.
• All placeholder text can be overwritten without breaking the layout.
• No third-party fonts, macros, or external links.
Once approved, I’ll drop it straight into daily use, so clarity and reliability are more important than elaborate graphics.
