Numeric Data Entry Transfer -- 2

I have a set of numeric records that currently live in a proprietary, custom-built database. I need every figure lifted out accurately and keyed into the destination sheets I will provide so I can run consistent reports. You will receive read-only access to the database interface (along with a short field guide that explains its layout) and a blank copy of the spreadsheet template that these numbers must populate.

Accuracy is the top priority—decimal placement, units and column order must match the template exactly. I will run spot checks, so an error rate above 0.2 % is unacceptable. Once complete, submit the filled spreadsheet plus a brief log of any ambiguous values you encountered and how you resolved them.

If you have previous experience extracting data from custom databases rather than standard SQL or NoSQL engines, note that in your proposal; it will help me gauge fit and turnaround expectations.
