Modern Word Template Redesign

My 15-page project-management questionnaire is currently just plain text. I need it rebuilt in Microsoft Word as a reusable template that feels modern and clean while remaining easy to fill out. Please retain the current colour scheme.

Here is what I’m after:

• Keep every question exactly as written, but introduce clear text boxes and tick boxes so people can type or check answers directly in the file.
• Add tables only where a structured grid will genuinely help—no need to force every section into rows and columns.
• Work in subtle icons and graphics to guide the reader, plus well-designed headings that create a logical hierarchy and break up long stretches of text.
• Apply consistent styles, light borders, and a muted colour palette so the whole document looks cohesive yet uncluttered.

Deliverable: a ready-to-use .docx file that maintains roughly the same length, prints cleanly in black and white, and lets me adjust content later through Word’s style gallery. I’ll check for correct text transfer, functioning form elements, tidy tables in the key sections, and an overall polished finish.
