Modern A4 Prescription Pad Design

I need a modern-looking prescription pad laid out on A4 that a doctor can print or send straight to the press. The page must carry all the usual sections—patient details, date, medication grid, signature line—while still feeling clean and contemporary.

Because there is no existing logo or visual identity, part of the job is to craft a simple, professional mark and choose a colour palette and typefaces that work for medical documents. Once we agree on the look, you’ll flow it consistently through the pad and supply:

• a print-ready PDF with bleed and crop marks
• the editable source file (Adobe Illustrator, InDesign or compatible)
• the small logo as a separate vector file for future use

Accuracy, legibility and alignment matter; margins must respect standard binding space, and the final PDF should be ready to hand to any offset or digital printer without adjustment.
