Mobile-First Client Intake Form Upgrade

I’m refreshing our website’s enquiry flow and need a streamlined, mobile-optimised intake form that any visitor can complete in 30 seconds or less. The form will sit on an existing page and collect 5–6 core details—Name, Company Name, Work Email, Phone Number—plus one key qualifying field, “Service Required,” pre-populated with “Support.” Everything should look clean on smaller screens because most users will arrive from their phones, yet still render perfectly on tablet and desktop.

Once a visitor submits, the data must be validated (required fields, correct email pattern, no junk text) and then routed securely to our company inbox as a new-client request. The sender should see an instant on-screen thank-you message confirming receipt.

Deliverables
• Responsive form UI styled to match our current branding
• Client-side and server-side validation for all inputs
• Dropdown or radio set for “Service Required” with “Support” as the listed option (flexible for future additions)
• Secure backend handler that emails the full submission to our designated address
• Confirmation/thank-you message shown after successful send
• Cross-browser and cross-device testing covering mobile, tablet, and desktop

Acceptance criteria
1. Form completes without errors on iOS Safari, Android Chrome, latest desktop Chrome/Firefox/Edge.
2. Invalid emails or empty required fields trigger inline prompts.
3. Successful submission delivers an email containing every field exactly as entered.
4. Page displays confirmation within one second of submit.
