What We’re Looking For
We are seeking a dedicated and experienced Licensed Supervisor, Care Management - RCT to join our team! The Licensed Supervisor, Care Management - RCT oversees a team who provide integrated, whole-person care within a managed care environment. The Licensed Supervisor, Care Management – RCT would be expected to work and be available at their remote location between the hours of 8:30am – 5:00pm and be flexible to travel within the community for meetings as needed. The role is responsible for the supervision of staff, ensuring quality service delivery, compliance with regulatory and accreditation standards, and the effective coordination of care for members with complex needs. In addition, this role provides clinical supervision and guidance, applying professional licensure to support care planning, clinical decision-making, and resolution of complex member cases.
On a typical day, you might:
Supervise staff, including oversight of day-to-day responsibilities, performance management, development, and time and attendance.
Ensure staff utilize established workflows to complete comprehensive assessments and care plans for members.
Establish and maintain a team-based, person-centered approach to care coordination that addresses members’ physical, behavioral, and social health needs.
Monitor team performance metrics, caseload distribution, and quality indicators to ensure effective service delivery.
Provide coaching, education, and guidance to staff to support continuous improvement and professional development.

Qualifications
Required:
Associate's degree (RN only) and three (3) years experience providing care management, case management, or care coordination to the MH/SU/IDD population; OR
Bachelor's degree (RN only) and three (3) years experience providing care management, case management, or care coordination to the MH/SU/IDD population; OR
Master's degree and fully licensed LCSW, LCMHC, LPA, LMFT, or RN and three (3) years experience providing care management, case management, or care coordination to the MH/SU/IDD population.
Fully licensed in the State of North Carolina as one of the following: LCSW, LCMHC, LPA, LMFT, LCAS or RN.
If applying with an Associate’s or Bachelor’s degree, you must be fully licensed in the state of North Carolina as an RN.
Must have a valid driver’s license.
Must reside within Trillium’s catchment area of North Carolina, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.
Must be able to travel within catchment as required.
Preferred:
Degree in Human Services field.
At least one (1) year supervisory experience with two (2) or more staff.
Experience with an MCO, Health Plan, or managed-care care management.
