Jumbo Postcard Questionnaire Design

I need a clean, eye-catching design for a jumbo-size postcard size as a questionnaire I can hand out for property presentation purposes. The goal is to a property seller fill out an easy form collecting quick insights for a 1min presentation so the card has to be simple, clean and easy to follow and fill out.

Front side
• Grab attention immediately with a concise headline, a short paragraph that frames the postcard as a “Questionnaire Card,” and a clear incentive or teaser for completing it.
• Visuals should follow. Logo in front top also.

Back side (the form)
• Include a mix of multiple-choice and open-ended questions—no more than eight in total—to keep the process quick.
• Provide big, easy-to-mark checkboxes and lined areas so handwriting stays legible.

Deliverables
– Print-ready PDF (CMYK, 300 dpi, 1/8-inch bleed) sized for USPS “jumbo” postcard specs
– Source file (AI, PSD, or InDesign) so I can tweak wording later
– A low-res JPEG mock-up for approvals
