IPDRG Coding for Patient Charts

I will be sharing a batch of electronic patient charts that require accurate IPDRG assignment. For each record, use only the discharge summary and the final diagnosis list to determine the correct grouping. The rule set comes from a country other than the Philippines, and I will supply the full guidelines along with sample completed records so you can see exactly how the groupings should look.

What you will receive
• A folder of charts in PDF format, each clearly labelled with a unique case ID
• The official DRG rule book for the specified country in searchable PDF form
• A template spreadsheet with the following columns: Case ID, IPDRG code, Brief rationale

What I expect back
• The completed spreadsheet with every case ID matched to one definitive IPDRG code, filled in exactly as the template shows
• Any coding questions flagged in a separate “Queries” tab so we can resolve them quickly
• Consistent application of the supplied rule set—no local or hybrid rules

Acceptance criteria
1. Every chart accounted for in the spreadsheet
2. Codes validated against the provided rule book without exception
3. No use of information outside the discharge summary and final diagnosis section

Once the first small batch is validated, the remainder of the charts will follow in rolling milestones.
