ICD-10 Inpatient Coding Project

I need a certified coder to assign accurate ICD-10 codes to a series of general-medicine inpatient charts. You will receive complete electronic health records—admission notes, progress notes, discharge summaries, imaging and lab reports—and return a coded file for each encounter.

The focus is strictly inpatient, so you will determine principal and secondary diagnoses, procedures, and any MCC/CC that affect DRG assignment. All coding must follow the latest Official Guidelines for Coding and Reporting as well as hospital-specific policies that I will share after award.

Deliverables
• One spreadsheet (or 837I flat file if you prefer) per batch, listing MRN, encounter dates, all ICD-10-CM and PCS codes, DRG, and coding rationale comments.
• Turnaround time of 48 hours for each 25-chart batch.
• Error rate below 2% when audited against AHA Coding Clinic guidance.
