E-commerce products promotion and spelling correction

I have a backlog of paper-based patient records that must be converted into clean, searchable digital data. Every document still sits in its original physical folder, so the first step is high-resolution scanning. Once scanned, the information—demographics, visit notes, medications, and any coded fields—needs to be keyed accurately into our secure database.

Accuracy and patient confidentiality are critical; any experience working under HIPAA or similar privacy frameworks will be a strong plus. I am happy to provide a brief data dictionary and remote database access so you can see the exact field layout before starting.

Deliverables
• PDF or TIFF scans of each record, named according to the supplied file-naming convention
• Complete data entry of all fields into the database with 100 % consistency checks against the originals

I will review a small pilot batch first; once that is approved we can proceed in larger tranches until the full archive is complete.
