Glassmorphic Servant Registration Form

I need a single-page servant-registration form for the Church of the Virgin Mary and Saint Mary Magdalene, built entirely in Arabic and flowing right-to-left. The form should open instantly on any device, and the layout must stay perfectly responsive from the smallest phone to large desktop screens; that adaptability is a top priority.

Visual style
• Modern, minimalist glassmorphism with a clear frosted-glass effect on key panels.

Core functionality
The page collects six fields:
1. Full name
2. Address
3. Date of birth

6. Confession father’s name

When the user taps “Submit,” the data is sent through Web3Forms directly to our designated email, so the solution stays completely server-free. A honeypot field plus hCaptcha keep bots out without adding friction for real visitors. The page must display an on-screen success alert as soon as Web3Forms confirms receipt, and a clear error message if anything fails.

• All text and placeholders in Arabic; ensure proper RTL alignment throughout.
