Form-to-PDF Data Capture Setup

I need a straightforward online form that gathers user input for data-collection purposes and pipes those answers straight into a predefined PDF file. The form itself only requires simple text fields—no checkboxes or dropdowns at this stage—yet the finished PDF must show the responses in the correct locations so it is ready for download or email immediately after submission.

You may build the form in plain HTML/CSS with JavaScript, PHP, or a trusted form-builder service, as long as the end result is a reliable, self-hosted solution that works across modern browsers. The generated PDF should preserve my existing layout and fonts and remain fully editable should I need to tweak the template later.

Deliverables

• Script or integration that merges each submission into my PDF template and outputs the completed file
• Brief setup notes so I can update the PDF or deploy the form on another server when needed
